IconX freaks me out, those icons jumping out at ya. Skeery!

I do not use IconX due to use of ObjectDock Plus and my needs, so I may be of limited assistance - so hopefully others will chime in.
The following are the items I would try:
1 - Set up IconX the way you want it, including selecting the "Run at startup" option, and then save your theme. Reboot the machine to test the "Run at startup" function.
Note - I do know that IconX needs to write a text file (".ini") to cache when Windows shuts down if the "Run at startup" is selected, and I have noticed that with some Anti-Virus programs (Norton, for one) - the script blocking feature will invoke an error message from Windows as Windows exits with something like "Unable to write text file", or similar. I have found that this is a one time only for a given theme, and that subsequent reboots do not invoke an error on exit.
2 - Look in one of the following to verify that a shortcut to IconX was created when you selected "Run at startup":
- "C:\Documents and Settings\All Users\Programs\Startup" folder
- "C:\Documents and Settings\USERNAME\Programs\Startup" folder
