Unable to deactivate on previous install

Hello all,

 

So I had an issue with my previous windows install and was forced to rebuild my OS.  So I'm using the same computer just a new OS build.

 

I would like some assistance from Stardock to remove my activation on the previous install.  I can provide whatever information is needed to prove it's the same pc.
