I notice that some icons on pinned programs on the taskbar return to the default application icon, I am unable to change the icon after this occurs.

Typically this means the OS icon cache is corrupted. Does the issue continue after signing out and in again?
The OS icon cache is not corrupt because when i switch off Start11 the icons return!.
I have resigned in , rebooted, updated and screemed at the machine since this issue began.
Each icon size has its own cache which means S11 may show issues when other things do not if the icons are not exactly the same.
That said, do the items with issues have anything in common? Stored on a network drive, or all universal apps etc?
They are all standard links (no alternate icon) however other applications also with standard icons do show. Is there any way for Start11 to refresh its cache?
Start11 doesn't maintain any cache, it uses the OS ones. If you get the wrong icon the most likely explanation is the OS is giving Start11 the wrong one.
If you right click on the pinned shortcut, what icon appears in the popup jump list?
Also if you change OS scaling factor to another size, does the problem continue?
