Why don't icons from Icon Packager don't show up fine in all open/ save as dialog properly?

I'm using Stardock IconPackager Enhanced 2.5.

When I apply the theme it looks ok in explorer but not in all dialogs.

Can I fix it?

it does this to me too everyonce and a while, reloading the icons normally fixes it for a while. I'd try to help you more, but I am afraid that I'll cause your computer to forget that it has a c:\ drive.

No problem, my C drive is used only for w98 which I didn't boot up a year now, I'm on XP Pro now, on drive E.

And the rebuilding icon cache, or rebooting won't help.
